IV services may be provided by experienced registered nurses or advanced practice providers. RN visits always include provider oversight.

Most common illnesses are expertly handled by our APPs. If you have a complex medical history or prefer a physician's expertise, you can select the physician upgrade.

What wounds can you repair?

We treat simple lacerations on extremities and torso. Complex wounds involving tendons, arteries, or sensitive facial areas typically require emergency care.

Do you sign school forms?

Yes. Our providers can sign standard State of Illinois school health forms and IHSA sports participation forms.

What injuries can you treat?

We evaluate sprains, strains, minor fractures, dislocations, and other musculoskeletal injuries. If emergency surgery is needed, we'll coordinate urgent transfer.

Urgent care treatment
Telehealth consultations

3. Medical Disclaimer

Our services are not a substitute for emergency medical care. If you are experiencing a medical emergency, please call 911 immediately.

The information provided through our platform is for general informational purposes only and should not be considered as a substitute for professional medical advice, diagnosis, or treatment.

4. Consent to Treatment

By booking a visit, you consent to receive medical examination, treatment, and diagnostic procedures as determined necessary by our healthcare providers. You have the right to refuse any recommended treatment at any time.

5. Payment Terms

Payment is due at the time of service unless otherwise arranged. We accept major credit cards and may bill insurance where applicable. You are responsible for any amounts not covered by insurance.

Cancellation Policy: Cancellations made less than 2 hours before a scheduled appointment may be subject to a cancellation fee.

6. User Responsibilities

Provide accurate and complete information
Inform our providers of all relevant medical history
Follow treatment recommendations or inform us if you choose not to
Make payment for services rendered

7. Limitation of Liability

Conciurgent Care and its healthcare providers are not liable for any indirect, incidental, or consequential damages arising from the use of our services, to the maximum extent permitted by law.
