Use your HSA or FSA card at checkout. Save your itemized receipt — it serves as documentation for your plan administrator.
This service is not covered by Medicare or Medicare Advantage and cannot be submitted for reimbursement. Medicare patients sign an Advance Beneficiary Notice (ABN) at the visit acknowledging out-of-pocket payment.
