Insurance & HSA / FSA

We do not bill insurance directly. We provide a superbill you can submit to your carrier for potential out-of-network reimbursement. HSA and FSA cards are accepted at checkout.

Medicare patients

This service is not covered by Medicare or Medicare Advantage plans and cannot be submitted for reimbursement. Patients with Medicare will be asked to sign an Advance Beneficiary Notice (ABN) at the visit.
